Application for War Risk Insurance for 1st Lieutenant William Wakeman Hackett, 121st Field Artillery (32nd Division) on December 5, 1917. He lists his mother, Carrie E. Hackett, the beneficiary for $10,000 if he should be killed or permanantly disabled. It was approved on January 7, 1918 at Camp MacArthur, Texas, where the 32nd Division was in training prior to departing for France.
