Notes Three-quarter seated view of Private David Samuels, Company D, 5th Wisconsin Infantry, wearing a mounted service jacket with the tape removed and a 6th Corps medal on his left breast. Samuels enlisted from Beaver Dam on January 4, 1864 as a recruit, transferred with all veterans and recruits to the Independant Battalion on July 13, 1864 and in September into Company C, 5th Wisconsin Infatry (Reorganized). He mustered out July 11, 1865.
